Summary
Per the document linked below to enable provider search for users all that is needed is read access to the following objects:

Care Provider Searchable Field
Care Specialty
Care Taxonomy
Healthcare Facility Network
Healthcare Payer Network
Healthcare Provider
Healthcare Provider Npi

However even with read access set some fields setup in the component editor will not show on the Provider Search component for all users. Addition Field Level Security (FLS) adjustments may need to be made to get the fields to show up as expected.

Workaround
Admins may need to review and extend FLS access to all profiles accessing Provider Search by marking the following fields as visible:

FacilityName

ProviderName

Specialty -> CareSpecialty.SpecialtyType

SubSpecialty -> CareTaxonomy.TaxonomyType

PlanType -> HealthcarePayerNetwork.NetworkType

IsAcceptingNewPatients -> HealthcareFacilityNetwork.PanelStatus

ProviderType -> HealthcareProvider.ProviderType

GenderRestriction -> HealthcareFacilityNetwork.GenderRestriction

NationalProviderIdentifier -> HealthcareProviderNpi.Npi
