Repro
1. Create a list view on contacts For example "XYZ Contacts"

2.Now for any existing Survey, try sending mass email from the survey builder using the "Send to List" option from Survey builder.

3.Choose Object :contact
List view "XYZ Contacts"
Recipient : "Contact.id"
Associated record : "Contact.id"

4. Click next, next, and then send a unique link.

5.Add "Survey Invitation and Responses" Related list to the contact layout.

6.Once the survey email is sent, then check the participant in "Survey Invitation and Responses" related records, it will link incorrect Survey invitation to the contact and hence the participant names will appear incorrect under a contact.

Workaround
None at this moment apart from using Process Builder or other methods of sending survey
