SFDO Case Management: Component error when editing Contact Lightning Record Page

Fixed in version Version 1.22
When attempting to edit the Contact Lightning Record page, a component error is displayed:
Message: Uncaught Failed to initialize a component [Definition does not exist on the client for descriptor:markup://sfa:outputNameWithHierarchyIcon. Client side caches have been cleared. Please reload the page.]
Callback failed: serviceComponent://ui.force.components.controllers.recordLayoutBroker.RecordLayoutBrokerController/ACTION$getRecordAndLayout
Go to "setup" -> "Lightning App builder" and click "View" Next to "Case Management Contact Record Home"
Notice the error
