Users may see the following error sometimes if they have a process builder flow that updates Employee Crisis Assessment record after it is created.
Repro
1. Log in as a System Admin
2. Create a process builder flow that updates the Assessment field on the Employee Crisis Assessment record after it is created.
3. Create Crisis Assessment record
