Unable to reorder items in the Actions & Recommendations component

Summary
Beginning in the Summer ‘19 release, you can reorder actions that appear in the Actions & Recommendations component in Lightning Experience.
From a dropdown menu on the action, Move Up and Move Down let you reposition the action within the top-pinned, bottom-pinned, or unpinned region. Salesforce tracks an action’s position in an Order field, and updates field values when there’s a change.
On limited occasions, updates to the Order field can occur out of sequence, resulting in the same value assigned to more than one action. When actions don’t have a unique value, they appear “stuck”, and you can no longer reorder actions in the list.
Repro
Open a record page with the Actions & Recommendations component, and add several actions.
From the dropdown menu, use Move Up or Move Down to reposition an action.
If the error condition occurs, selecting Move Up or Move Down has no effect.
This error is not easily reproducible. It sometimes occurs when API or process automation tools are used to create or update actions in the list.
Workaround
The workaround is to reset order values for actions on the list using Workbench or the API. Assign each action a unique sequential value from 1 to N, where N is the total number of actions for all regions.
