All care plans subtab does not reflect changes made to individual care plan cases

Summary
The All Care Plans subtab in the Patient tab doesn’t reflect changes made to a care plan’s case.

Repro
1. In the App Launcher, select Health Cloud - Lightning Console.
2. Go to the Patients tab.
3. Choose any patient.
4. Go to the All Care Plans subtab.
5. Open an existing care plan.
6. Click Edit.
7. Update the case Status and click Save.
8. Navigate back to the All Care Plans subtab.
9. The care plan still shows the old status.

Workaround
Refresh the All Care Plans tab
