Salesforce for Outlook - Side Panel display "You're not connected to Salesforce" when authenticating via SSO

Summary
If a Salesforce org is set up to use Single Sign-On (SSO) with SAML, the Side Panel may show the following message, effectively preventing the user from using it:
"You are not connected to Salesforce"
The AddinTrace.log has a line where it shows a PasswordChange is required
Here is an example:

Repro
After upgrading or installing SFO v3.x or higher, open Outlook and login to SFO and complete the wizard, The Side Panel will load and in a few second the message is displayed.
Workaround
#1 Reset user's password from the Salesforce org | user profile
#2 Alternatively, remain on a lower SFO version such as SFO v2.9.3 and hold off from upgrading at this time. We are working on making a new SFO version available that does not exhibit that behavior.
