Repro
Steps to Reproduce

1. Login as a System Admin with the HealthCloud Admin permission set.
2. Navigate to Setup | Develop | Custom Metadata Types and click Edit next to HcFeatureDriver.
3. Uncheck the "Active" box to deactivate it and Save the changes.
4. Clone it and update the "Setting Value" on the cloned version to have the value "Contact" and make sure the Active box is checked..
5. Update the "Patient Layout" on the Contact object to include the "PatientProfile_Page" as a Custom Console Component.
6. Go to Setup | Develop | Custom Settings and click Manage next to Card View Dropdown
7. Click New to create a new Card View Dropdown with the following settings:
Name: Relationships
Category Name: Relationships
Tab Type: Primary Subtab
Subcategory Name: Household
Page Type: VFpage
URL: /apex/HealthCloudGA__HcRelationshipPage
Default Subtab: checked/enabled
8. Go to the Health Cloud Console
9. Go to Patients tab and open a Patient
10. Click the Relationships subtab

Workaround
No workarounds at this time. You may want to consider using the out of the box meta data for HcFeatureDriver.
