Repro
1. Log in as a System Admin on an Action Plan enabled org
2. Create a new Action Plan Template for the Opportunity object and Publish it
3. Create the following process and activate it:
Object: Opportunity
Start the process: when a record is created or edited
Criteria for Executing Actions: Conditions are met
Set Conditions: Field: Opportunity.IsWon | Operator: Equals | Type: Boolean | Value: True
Immediate Action: Create a Record
Record Type: Action Plan
Set Field Values:
Action Plan Template Version ID | ID | Id of your Action Plan Template Version
Action Plan Type | Picklist | Industries
Action Plan Name | String | Test
Owner ID | Field Reference | Opportunity.OwnerId
Action Plan Start Date | Field Reference | Opportunity.CloseDate
Target Record ID | Field Reference | Opportunity.Id
4. Create an Opportunity
5. Set the Stage to 'Closed Won'
6. Save the Record

No workarounds at this time. However, you can create a Action Plan using the Action Plans Lightning component.
