Users are not getting "This request is taking a long time" alert and email upon completion of long running requests

Certain actions can end up taking a long time to process, such as changing user Roles and Profiles where a lot of record sharing is involved.
Normally, this will cause the system to bring up an alert stating "This request is taking a long time" and the user would receive an email alert when it was completed.
In some Salesforce instances, instead of getting this error message, the progress spinner will continue to stay on screen indefinitely and no alert nor email will be activated.
Attempt any action that would normally cause the alert to appear.
