Lightning for Outlook: On-Premises Exchange environments with special configurations do not load "Add Email" icon

This only applies to customers with On-Premises Exchange servers that have special Exchange configurations where the subdomain for the EWS URL is different from the Metadata URL entered in Salesforce Lightning for Outlook Settings:
Lightning for Outlook won't load the "Add Email" icon. This fails due to a EWSURlValidator check performed by Salesforce.
