Spring '14 - Internal server errors with stack trace id -1744735763

Summary
An issue with the “Managed Package Shared Code Cache” is causing Internal Server Errors to occur for some organizations that use managed packages. Because some application servers may not be exhibiting the error, requests that land on those servers will succeed.
Internal Server Errors with stack trace id -1744735763 due to NullPointerExceptions have been reported by a number of customers:

Repro
This issue can be reproduced in various scenarios, typically when code found in managed packages is evaluated.
Workaround
It seems this issue is mostly intermittent, so when the same steps are retried, the request completes successfully.
- 20th February: R&D are working on a fix for this issue at present.
- 22nd February: Fix was rolled out to Spring '14 sandboxes and NA1 (currently on Spring '14)
