When navigating to the Intake page, getting error on Case page components (both Inbound Referral and Outbound Referral)

When creating a Case Plan, clicking the save button results in a component error "Cannot read property 'newCasePlanSuccess' of undefined", which appears on screen before the newly created Case Plan record loads.
